Liability waiver

Read and sign below. This is the same agreement summarised in our Terms of Service.

1 · Assumption of risk

Basketball involves physical activity and an inherent risk of injury, including sprains, fractures and contact injuries. I confirm the athlete is physically able to participate and that I have disclosed any relevant medical condition to ShigHoops LLC.

2 · Release

To the fullest extent permitted by California law, I assume the ordinary risks of athletic participation and release ShigHoops LLC from claims arising out of those ordinary risks. Nothing here releases liability that cannot lawfully be released.

3 · Medical care

If I cannot be reached in an emergency, I authorise ShigHoops LLC to seek emergency medical care for the athlete at my expense.

4 · Cancellation

Sessions need 24 hours' notice to reschedule. Memberships can be cancelled or changed with 7 days' notice before the next billing date.

5 · Media

Clips may be recorded for coaching feedback. Nothing identifying a minor is published publicly without separate written permission, which I may withdraw at any time.
